Supported Living

We are a supported living service providing support to bespoke tenancy service users in the comfort of their own homes. We are driven and passionate about enabling those we support to build resilience, capacity, skills and coping mechanisms thereby allowing them to live as independently as possible in their own homes.

We provide a safe personal care, supported living, palliatives care, semi independent living, home care services, patient escort to the appointment or social engagement, food preparation and assistance with meals with a focus on healthy eating, assistance with medication management, mobility and falls prevention management, respites services, live in care.

All our staff share this belief and are supported to ensure that our mission is reflected in every aspect of the care and services provided. Our aim is to safeguard the privacy and confidentiality of clients and to enable the clients to maintain their personal independence, have informed choices and opportunities to enjoy and contribute to society.

  • Support with accessing the community

  • Food preparation and assistance with meals with a focus on healthy eating

What We Offer

We also offer a community step down service for mental health patients further supporting their transition and rehabilitation into the community. We are led by the needs of those we support.

We are highly skilled, competent and knowledgeable, all our staff undergo robust vetting processes to ensure that they are able to deliver a great service.

We adhere to strict organisational polices and procedures which ensure that those we support are safeguarded, enabled, empowered and are able to practice their right to self- determination. We collaborate with those we support and assist them to identify the best accommodation to meet their needs.
